Before submitting a formal application for accreditation, please download and read: ISAS 1 The Conduct of UKAS Assessment for the Imaging Services Accreditation Scheme

To apply, please download and complete all application documents listed below and submit with the application fee of £1,200 plus VAT.  Cheques or purchase orders are accepted. Please note that a handwritten signature is required on the application documents.
